Symptom narrative

03/03/26- 03:38pm received 3 vaccines (Hep A, MMR. and Varicella). 03/02/26 - around 11:00pm child started with fever of 103. 03/03/26 - Child pulling at ears, congested, fever, and denies wheezing/SOB. 03/03/26 - Around 08:59am had episode of possible febrile seizure x 1 minute, staring and shaking, not responding to mom, crying/irritable. Nurse recommended that the mother take the child's temperature and treat the fever and go to the ER. Mother and father took child to (name withheld). Nurse called (name withheld) and gave them report on the child and gave them and ETA of the child's arrival.
