Symptom narrative

Staff administered a Spikevax Moderna 0.25Ml vaccine to a 17 year old patient. Patient received a smaller dose than recommended. No adverse effects reported. Patient and family were notified. Patient's pediatrician was also notified and is recommending patient gets a full/correct dose of Pfizer covid 19 vaccine in 8 weeks.
