Symptom narrative

Pt tolerated PCV20 and MMR vaccines well. Havrix or Hiberix caused significant hives (approximately 2.5in circle) at site; this RN monitored pt for 30 min after to ensure reaction did not progress. No nursing interventions needed except constant then q5 min visual assessment and attempted ice pack at site. Pt only exhibited hives and urticaria; mom chased pt around clinic while he played during post-vaccine observation. No increased WOB, retractions, SOA, or wheezing/stridor noted at any time. Hives improved by end of 30 min observation. Mom instructed to give Benadryl once home and continue to monitor child. DON made aware.
