Rash aprox 8cm round, firm, red and warm lesion left thigh at site of previous vaccines. no pain reaction to palpation. c/o itching. no limping or pain symptoms. Treated with hydrocortisone 2.5 % topical ointment TID PRN itching x 3-5 days.
