Vaccine administration error: Patient received Measles, Mumps, and Rubella (MMR) Vaccine MMR Lot# Y00411 7 that had expired on 02/12/2026. The error was discovered on 03/03/2026, after administration. Parent/guardian will be notified. No adverse reactions observed at the time of administration. Revaccination recommended =28 days after the expired dose was given.
