When pt presented in clinic with staff no outside immunization records were given. Nothing pulled up in database on 2/24/26 so therefore screening nurse RN administerred the following: tdap, flu, mcv4, hpv,ipv,,mmr, hep a, varicella that day.. Much later clinic faxed records from location post vaccination date of service and on 3/2/26 historicals were entered into system. Upon inspection of new system record it shows the following vaccines as RED X: tdap given on 2/24/26 was given too soon for 3rd dose, flu shot was given 2/24/26 and not needed as pt received dose on 12/9/26 in state unbeknown to screening nurse as records were not presented at date of service. 2nd dose of mcv4 not needed pt over age 17. HPV 3 rd dose 2/24/26 given too soon. 3rd dose of polio 2/24/26 given too early. 3rd mmr given on 2/24/26 not needed. 2nd hep a given on 2/24/26 was too early. 3rd varicella given on 2/24/26 not needed.
