Symptom narrative

no symptomatic adverse events; that the product was exposed to a temperature excursion as well; administration of expired PNEUMOVAX-23 to a patient on 5-MAR-2026.; This spontaneous report was received from a register nurse and referred to a 68-year-old female patient. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 05-Mar-2026, the patient vaccinated with Pneumococcal Vaccine, Polyvalent (23-valent) (PNEUMOVAX 23) injection (0.5 ml, once, lot #X027348, expiration date: 24-Jan-2026, strength, dose number and route were unknown) for prophylaxis (Expired product administered). There were no symptomatic adverse events. The vaccine was exposed to a temperature excursion as well (Improper storage of product in use). The stored temperature was 48.6 Fahrenheit (F). The time frame was 30 minutes. There was no previous temperature excursion.
