14-month-old female, experienced a fever the first day; Medical Assistant/Manager reports another medical assistant accidentally used BEYFORTUS 100mg (Sanofi) to reconstitute PROQUAD, she states the Medical Assistant thought it was STERILE DILUENT. Patient was called and they were informed the patient, a; This spontaneous report was received from other health professional and refers to a 14-month-old female patient. The patient's concurrent conditions included Pneumonia. The patient's medical history and concomitant therapies were not reported. On 27-Feb-2026, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), lot #Z014435 has been verified to be a valid lot number, expiration date: 11-Mar-2027, administered at a dose of 0.5 mL for prophylaxis. The vaccine administered was accidentally reconstituted with nirsevimab (BEYFORTUS) lot #UK429AA, expiration date: 31-Dec-2027 (dose, indication, anatomical site of injection and route of administration were not reported) (Product preparation error). On 27-Feb-2026 (reported as the first day), the patient experienced a fever (pyrexia). At the reporting time, the outcome of the event was unknown. The causal relationship between the event of fever and Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live was not provided.
