Patient presents to clinic 12/12/2025 in need of Hep B vaccine for 2nd dose in series. Patient previously recieved 1st documented dose 09/24/2025. Notified on 03/05/2026 that there was an error in giving the vaccine to said client. According to information presented on date of visit, the patient was in need of Hep B series #2. There was not any supporting documentation that stated otherwise. After investigation, the previous Hep B vaccines were entered on 02/13/2026. On 02/13/2026, 5 historicaly Hep b vaccines were entered into system by a unknown person.
