Symptom narrative

Pt came in for vaccines, pt was due for 4 vaccines including the Hep A, pt received Hep A peds dose, notified nurse, Nurse asked if pt came in to get second dose, I had let her know no. Nurse asked me to call pt to come before 6:00 for her second dose. Called pt to come in before 6:00, pt did not answer notified nurse, nurse asked me to document that she would need a full dose when she comes in.
