Symptom narrative

An early dose of MMR vaccine was given 03/06/2026 during patient's 2 month old Wellness check. She was also given the other vaccines listed above (Pentacel, PCV20, Rotavirus, and Hepatitis B). It was given due to concern for possible measles exposure when they travel out of state on March 16th. They will be visiting family members. Patient remained in the clinic for about 15 minutes post vaccine administration without any observed adverse effects.
