Incomplete dose given to a patient/ administration most of the vaccine was not administered; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of incomplete dose administered in a adult male patient who received Herpes zoster (Shingrix) for prophylaxis. Concurrent medical conditions included immunocompromised. On 22-FEB-2026, the patient received Shingrix. On 22-FEB-2026, an unknown time after receiving Shingrix, the patient experienced incomplete dose administered (Verbatim: Incomplete dose given to a patient/ administration most of the vaccine was not administered). The outcome of the incomplete dose administered was not applicable. Additional Information: GSDK receipt date- 27-FEB-2026 The reporter stated that during the vaccine administration, most of the Shingrix dose was not administered due to an issue with the needle. No product quality complaint (PQC) was identified. The pharmacist asked whether the dose should be repeated. No further details were provided.
