Symptom narrative

pt came to pharmacy on 3/6/26 and stated that his right arm was in pain and he was not able to move his arm outwards since a previous vaccine on 2/4/26. during the initial visit the pt stated the technician initially placed the vaccine into the arm, but nothing came out. the pt also stated the technician pulled the vaccine out of his arm and then re-injected the vaccine into a different location of his arm and then injected vaccine. the pt expressed his concern that he felt the injection sight seemed to be a little higher than normal. the pt woke up the next day and stated his arm was sore and he was not able to move it outwards. the pt stated these symptoms got worse and went to urgent care on 2/13/26. urgent care told pt to go to ortho if he doesn't feel better within 10 days. pt went to ortho on 2/19/26 and they gave him a steroid injection and medrol dose pack. the pt stated he received some relief from treatment, but relief only lasted a few days. dr diagnosed pt with inflammation. pt is scheduled for an mri on 3/13/26 and follow up iwth dr on 3/24/26
