Symptom narrative

This morning at 0800, patient received the below vaccine booster in his left arm. Approximately 30 min later, he reported swelling, itchiness, and redness to the injection site as well as a swollen bottom lip, he was coughing and had an itchy throat. He stated that he ?did not feel right?. His neck and face were also red. At approximately 1030, patient was transported to [withheld] by [withheld]. Patient has been triaged and is awaiting further treatment.
