Patient called clinic 03/05/26. States she had no problems after received Tdap injection 02/27/2026 until about 5:00 pm 03/04/2026. On the phone patient stated that her arm started swelling around where she received the injection about the size of a half baseball and was red/warm and hurting. Patient decided to go to emergency department. ER report physical exam narrative states "Irregularly shaped area of warmth, erythema to left upper arm. Area is indurated but there is no underlying fluctuance. No lymphatic streaking. Moderate tenderness to palpation." Report also states, "No fever. On arrival to the ED, she is mildly tachycardic but otherwise vitals are within normal limits.". "CBC is unremarkable except for minimally increased immature granulocytes. Will go ahead and treat as presumptive cellulitis. This time blood makes more sense with the injection occurring a week ago and warmth and redness starting over the last few days. Would expect an allergic reaction to have started closer to the time of injection." ER notes show Dexamethasone 10 Mg X 1, Ceftriaxone 1 gm X1, and sent home with prescription for Bactrim DS 800-160 mg tablet.
