Symptom narrative

Lot Number X004913; the 50-year-old patient received 1st dose of VAQTA on 6/2009 (adult dosage administered) and 2nd dose given on 10/18/2023(pediatric dosage administered); This Spontaneous report was received from a Nurse and referred to a 50 -year-old patient of unknown gender. The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. In Jun-2009, the patient was vaccinated with the first dose of Hepatitis A Vaccine (VAQTA)(1dosage form, total, Asked But Unknown Batch/Lot Number, adult dosage) for prophylaxis. On 18-Oct-2023, the patient was inadvertently vaccinated with the second dose of Hepatitis A Vaccine, Inactivated, Pediatric, 25 u/0.5mL (VAQTA) (0.50 mL, total) (Batch/Lot Number reported as X004913 is an invalid lot# for Hepatitis A Vaccine, but it is a valid lot# for JANUMET XR; Expiration date reported as 28-Jul-2024, pediatric dosage) for prophylaxis (Inappropriate schedule of product administration, Product lot number issue).
