another Shingrix adjuvant was used instead of the supplied adjuvant; another Shingrix adjuvant was used instead of the supplied adjuvant; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate preparation of medication in a 63-year-old female patient who received Herpes zoster (Shingrix) (batch number 59PX7, expiry date 26-AUG-2027) for prophylaxis. On 03-MAR-2026, the patient received the 2nd dose of Shingrix. On 03-MAR-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: another Shingrix adjuvant was used instead of the supplied adjuvant) and inappropriate dose of vaccine administered (Verbatim: another Shingrix adjuvant was used instead of the supplied adjuvant). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
