Symptom narrative

Patient called asking to know what vaccine she received on her right arm as her right arm been red at spot she got vaccine on 3/3 and been red for 3 days. Pt was helping her mom with plants and the leaves touched at vaccine spot which was red and now arm is red, welt, bigger and hurts. Pt already had called her provider who suggested her to go to clinic today.
