Patient was her for her 9 year well child check up and HPV vaccine along with her older brother who was here for his 16 year well child check up, MCV4 and Bexsero vaccines. Medical assistant giving vaccines gave patient the Bexsero vaccine instead of the HPV vaccine and gave patient's older brother the HPV vaccine with the MCV4.
