Patient received a Pediatric dose of Hep B instead of an adult dose on 11/4/2025, it was just discovered on 3/2/26 by a travel Dr. Patient was notified and told she needed to come in for a Hep B Adult. She came in and received an adult dose of Hep B on 3/4/2026.
