we had an error and a patient was given only one vial of the adjuvant suspension component; Inappropriate dose of vaccine administered; This non-serious case was reported by a physician via call center representative and described the occurrence of inappropriate preparation of medication in a 53-year-old female patient who received Herpes zoster (Shingrix) (batch number K953J, expiry date 17-JUN-2027) for prophylaxis. On 02-MAR-2026, the patient received Shingrix. On 02-MAR-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: we had an error and a patient was given only one vial of the adjuvant suspension component) and inappropriate dose of vaccine administered (Verbatim: Inappropriate dose of vaccine administered).
