A 50-year-old female patient (unknown if pregnant) received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), as dose number uknown, single (Batch/Lot number: unknown) for immunisation. The patient's relevant medical history and concomitant medications were not reported. The following information was reported: FEELING HOT (medically significant), outcome "not recovered", described as "heat to the area"; PARAESTHESIA (medically significant), outcome "not recovered", described as "tingling"; HYPOAESTHESIA (medically significant), outcome "not recovered", described as "numbness"; PAIN IN EXTREMITY (medically significant), outcome "not recovered", described as "pain with like a zapping sensation that went through the palm of her fingers, the palm of her hand into a fingers"; JOINT SWELLING (medically significant), outcome "not recovered", described as "but then she also started to developed lumps and bumps in her(patient) wrist area/some were on the (her) knuckles"; PERIPHERAL SWELLING (medically significant), outcome "not recovered", described as "swelling got worse it spread to the front of the leg too, it was lower leg/lump on her leg/painful bump that started on her leg"; CARPAL TUNNEL SYNDROME (medically significant), outcome "unknown", described as "went through neurologist was told had a mild carpal tunnel but per caller the carpal tunnel would not make her disable like that"; TENDONITIS (medically significant), outcome "not recovered", described as "found some severe inflammation around tendons"; ARTHROPATHY (medically significant), outcome "not recovered", described as "wrist are very week"; LIMB MASS (medically significant), GRANULOMA SKIN (medically significant), outcome "not recovered" and all described as "the lumps were all different kind sizes, some were on knuckles, side of the wrist, some are still there, some are hard, some are soft, some like very granular"; MOVEMENT DISORDER (medically significant), outcome "recovering", described as "was not able to carry things, not able to brush her own teeth herself, because could not reach or bend her wrist"; ERYTHEMA (medically significant), outcome "not recovered", described as "redness". The patient underwent the following laboratory tests and procedures: Inflammatory marker test: Negative; Magnetic resonance imaging: Severe inflammation around tendons; Magnetic resonance imaging neck: Inaudible disease; Ultrasound scan: Negative, notes: Blood clot ruled out. Therapeutic measures were taken as a result of feeling hot, paraesthesia, hypoaesthesia, pain in extremity, joint swelling, peripheral swelling, carpal tunnel syndrome, tendonitis, arthropathy, limb mass, granuloma skin, movement disorder, erythema. Clinical course: Patient had a painful bump that started on her leg and it had swelling and redness and heat to the area, but then she also started to developed lumps and bumps in her wrist area, and that caused swelling, tingling and numbness and pain with like a zapping sensation that went through the palm of her fingers, the palm of her hand into a fingers. Patient stated after receiving Prevnar 20 within 24 hours noticed the lump on her leg, the swelling got worse it spread to the front of the leg too, it was lower leg and got ultrasound , ruled out a blood clot, the swelling went down, but could still feel the lumps, it took weeks for the lumps to go away. but similar lumps started appearing on her wrist, and fingers stared hurting/palm started hurting, wasn't able to make a fist. The swelling in the hand is still there, the doctor's could not figure out what it was because the lumps were all different kind sizes, some were on the her knuckles , some were on the side of the wrist, some are still there, some are hard, some are soft, some like very granular, like there is a texture to it, hand started getting swollen, tingling , numbness, zapping , was not able to carry things, not able to brush her own teeth herself, because could not reach or bend her wrist, went through neurologist was told had a mild carpal tunnel but the carpal tunnel would not make her disable like that. She had swelling on the wrist was compressing on her nerves, could not dress herself could not feed herself, "driving was particularly crazy she had to wear the thickest gloves or layered up on multiple layers so she could grip steering wheel to get herself to work, she couldn't write anything" after seeing multiple doctors, was put on Prednisone 20, helped a little but still wasn't able to make a fist, had MRI of her cervical, they found " inaudible" disease but all the inflammatory markers were negative. They did hand MRI and found some severe inflammation around tendons so went to the orthopedist, they gave her shots in her(patient) wrist, which helps her move her wrist , able to carry things again, not as much as she used to but her hand tremble now and the wrist are very week but able to function, but still with a lot of pain numbness and tingling still there. No follow-up attempts are possible. Batch/lot number is not provided, and it cannot be obtained.
