guessing that somebody used 2 Shingrix diluents alone.; guessing that somebody used 2 Shingrix diluents alone.; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate dose of vaccine administered in a patient who received Herpes zoster (Shingrix) (batch number N3HL7, expiry date 17-JUN-2027) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced inappropriate dose of vaccine administered (Verbatim: guessing that somebody used 2 Shingrix diluents alone.) and inappropriate preparation of medication (Verbatim: guessing that somebody used 2 Shingrix diluents alone.). The outcome of the inappropriate dose of vaccine administered and inappropriate preparation of medication were not applicable. Additional Information: GSK Receipt Date: 03-MAR-2026 Nurse noticed that one Shingrix box that had been used by somebody, there were only 2 brown tops vials left, guessing that somebody used 2 Shingrix diluents alone. The healthcare professional believes that one potentially match could be lot number K953J that corresponds to a diluent green top vial. If Nurses assumptions were okey, it looks like someone received 2 adjuvants, so obviously the dose was invalid.
