A 60-year-old female patient received BNT162b2 (BNT162B2), on 22May2021 as dose 1, 0.3 ml single (Batch/Lot number: unknown) and on 11Jun2021 as dose 2, 0.3 ml single (Batch/Lot number: unknown) for covid-19 immunisation. The patient's relevant medical history included: "anxiety", start date: 05Jun1997 (unspecified if ongoing); "elevated blood pressure", start date: 11Nov2016 (unspecified if ongoing); "trigeminal neuralgia", start date: 11Jun2020 (unspecified if ongoing); "depression" (unspecified if ongoing). There were no concomitant medications. The patient was hospitalized for costochondritis (start date: Nov2025). The events "costochondritis/costochondritis flare up/chest pressure/right side rib pain that wraps around to my thoracic and shoulder" required physician office visit and emergency room visit. Therapeutic measures were taken as a result of costochondritis which included currently being in physical therapy, taking muscle relaxers, anti-inflammatory and prescription meds. Clinical course: It took more than five years for the patient to receive a diagnosis. The diagnosis was made during a hospitalization in November 2025 for chest pressure, at which time a rhinovirus infection triggered a costochondritis flare. Prior to this, there had been numerous visits to specialists, emergency rooms, and urgent care facilities without a definitive diagnosis. She experienced persistent right-sided rib pain that radiated to the thoracic region and shoulder and was accompanied by abdominal discomfort. None of these symptoms were present before receiving the COVID vaccine. The pain significantly interfered with daily activities, including driving, sitting, standing, twisting, exercising, cooking, and cleaning, ultimately affecting every aspect of her life. Previously very active, she found that the condition severely limited her ability to function as a mother, wife, and grandmother. Due to the severity and persistence of the pain, she was forced to stop working full time and was later approved for disability benefits. The chronic pain also contributed to worsening anxiety and depression. The patient was not taking any other medications within 2 weeks prior to the event onset.
