Symptom narrative

On Friday 03/06 parents noted a rash on patient's left leg, around injection site. Temp of 101.2. Rash was not itchy or painful. Evaluated in clinic on Saturday 03/07. Diagnosed with cellulitis and prescribed cephalexin 350mg BID x7 days. Parents brought child back into clinic on Tuesday 03/10 due to the rash looking differently than it did on 03/07. Dr noted hives and redness around injection site. Center displaying a linear bruise. Dr recommended discontinuing the antibiotics and discussed with family filing VAERS report.
