Provider ordered MMR, varicella, Hep A. Nurse administered proquad (MMR/varicella combo), Hep A, varicella. Nurse manager advised nurse of adverse event, nurse advised provider and mom was contacted to advise of event. Questioned how child was after vaccines, mom stated fine.
