On 05-Mar-2026, the patient started therapy with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live lyophilizate (PROQUAD) 1 dose (total) with Batch/Lot Number: Z014409 and expiration date: 10-Mar-2027) as prophylaxis. The vaccine had a temperature excursion on 20-Jan-2026 (Product storage error).
