A pharmacist reported that a 23-year-old male received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization and only received half a dose. No medical history or concomitant medications were reported. On 15-Feb-2026, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number 947354, expiration date 30-Mar-2028, and NDC number 43528-003-05)] intramuscularly in the right deltoid. While administering the first dose of HEPLISAV-B, a good amount leaked out so the patient most likely only received half a dose, if that. The reporter stated that when the needle cap was pulled off, they must have twisted the cap in a way that made it come loose, causing the product to leak. No side effects were noted.
