Symptom narrative

15 Year Old Patient Received Two Doses of HEPLISAV-B; Initial report received on 23-Feb-2026. A nurse reported that a 15-year-old male received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization. No medical history or concomitant medications were reported. On 11-Sep-2025, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number 946908, expiration date 21-Dec-2027, and NDC number 43528-003-01)] intramuscularly in the left deltoid. On 13-Oct-2025, the patient received dose 2 of HEPLISAV-B (lot number 946908, expiration date 21-Dec-2027, and NDC number 43528-003-01) intramuscularly in the left deltoid. The patient received two doses four weeks apart. The patient was 15 years old when he received HEPLISAV-B. The patient was due for vaccination and HEPLIAV-B was accidentally given for hepatitis B prevention. No adverse events were reported. No other information was available.
