Administered HEPLISAV-B IV Instead of IM; Initial report received on 13-Feb-2026. A pharmacist reported that a 75-year-old female received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization intravenously instead of intramuscularly. No medical history or concomitant medications were reported. On an unknown date, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number 943888, expiration date 14-May-2026; and NDC number 139527430013)] intravenously instead of intramuscularly. The patient was monitored for 45 minutes after the dose was given. The patient had no symptoms verbalized during or after administration. No other information was available.
