Symptom narrative

On 2/23/26, Family Health Center experienced a power outage which caused a 1.5-hour temperature excursion. The DPH advised not to use the open vial of polio vaccine, but it did not get discarded right away. This dose was given from that vial. DPH was contacted immediately and advised risk of harm to patient very low. Patient will need to repeat dose as this dose is considered invalid. No adverse symptoms have been reported.
