Symptom narrative

During the visit, vaccines for an 11-year-old patient were ordered and signed by the provider. The patient s grandmother had previously taken the patient to another clinic for his 11-year well-child check; however, this information was not reported during the visit. The vaccines were discussed with the family, and the provider reviewed, ordered, and signed the immunizations. At the time of administration, the patient s vaccination record indicated that the patient was up to date, and there was no indication that the 11-year vaccines had already been administered at another clinic. After the visit, the patient developed a rash. The nurse subsequently reviewed the patient s immunization record in database and identified that the patient had previously received the 11-year vaccines at a different clinic. The situation was discussed in a meeting with management. It was also noted that I have not yet received formal training on how to verify or add new patient immunization information in database. Verification of immunization records and vaccine orders are completed based on the provider s review and signed orders. Vaccines were administered according to the provider s orders and the information available in the patient s record at the time of the visit.
