Symptom narrative

Patient came to the pharmacy to get 3 vaccines (chickenpox, hepatitis b, and hpv). Patient requested a certain type of hepatitis b vaccine therefor pharmacist administered 2 vaccines (chickenpox and hpv) first and left the room briefly to retrieve the third vaccine (hepatitis b) that patient requested. When pharmacist came back, patient had fainted and fell off his chair. 911 was called and patient was checked on. All vitals were normal.
