Symptom narrative

Pt received MMR II vaccine (Merck) at approximately 7:30 PM. Within approximately 10 minutes of vaccination she began feeling markedly fatigued and developed bilateral temporal headache and nausea. Within approximately 30 minutes of the injection she began vomiting. Approximately 45 minutes after vaccination she developed shortness of breath/air hunger. Patient was taken to urgent care for evaluation. Pulse oximetry fluctuated between the 80s and 90s, with the lowest reading observed at 84%, though it would quickly return to the 90s. Physician evaluation reported normal heart and lung exam and no visible throat swelling. Patient was treated with ondansetron (Zofran) for nausea and discharged with monitoring instructions. Headache, fatigue, and intermittent shortness of breath/air hunger continued for approximately 24 hours following vaccination and then gradually improved.

Allergies

Previous allergic reaction to TDAP vaccine at the age of 7. Cat allergy and seasonal allergies.

Lab data

3/9/26 Patient was taken to urgent care for evaluation. Pulse oximetry fluctuated between the 80s and 90s, with the lowest reading observed at 84%, though it would quickly return to the 90s. Physician evaluation reported normal heart and lung exam and no visible throat swelling. Patient was treated with ondansetron (Zofran) for nausea and discharged with monitoring instructions. She will f ollow up with PCP tomorrow.
