Symptom narrative

No additional AE; Caller reported AE on behalf of a patient who was inadvertently administered PROQUAD and VARIVAX concomitantly on 03/03/2026. Caller reported that they patient was expecting to receive 1 dose of VARIVAX and 1 dose of MMR-II.; Caller reported AE on behalf of a patient who was inadvertently administered PROQUAD and VARIVAX concomitantly on 03/03/2026. Caller reported that they patient was expecting to receive 1 dose of VARIVAX and 1 dose of MMR-II.; This spontaneous report was received from a physician assistant (reported as medical assistant) and refers to a 13-month-old male patient. The patient's concurrent conditions included fever, and illness (unspecified). Concomitant medications included hepatitis a vaccine inact (HAVRIX) and two topical creams (unspecified).The patient's medical history was not reported, previous drugs reactions and allergies were not reported. On 03-Mar-2026, it was stated that the patient was expecting to receive a dose of varicella virus vaccine live (oka-merck) (VARIVAX) and a dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live lyophilisate and solvent for solution for injection (M-M-R II; however, he was inadvertently administered with a dose of varicella virus vaccine live (oka-merck) (VARIVAX) at a dose of 0.5 milliliters (mL) administered subcutaneously as prophylaxis (Lot No. Z010437 has been verified to be a valid lot number for [varicella virus vaccine live (oka-merck)], expiration date reported as 18-Jul-2027, but upon internal validation established as 18-Jun-2027) (anatomical location of administration and vaccination scheme frequency were not provided) and with a dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA [recombinant Human albumin]) (PROQUAD) at a dose of 0.5 milliliters (mL) administered subcutaneously as prophylaxis (Lot No. Z011862 has been verified to be a valid lot number for [measles, mumps, rubella and varicella (oka-merck) virus vaccine live], expiration date reported and upon internal validation established as 13-Jan-2027) (anatomical location of administration and vaccination scheme frequency were not provided); both reconstituted with sterile diluent, administered subcutaneously (Lot No., expiration date and indication were not provided) (accidental overdose) (wrong product administered).
