on 3/11 patient received a vaccine and said she was fine after shot, got up to head out of vaccine room. Pt fainted and fell to ground. I talked to patient, had her remain on the ground and asked her name, which she responded with pt. States she hit her head lightly. I asked if she had fainted from vaccines before, she said no. She said no she had not eaten all day when I ask. Gave patient some water and crackers. Offered to call EMS to have her checked out. She declined. Did have front end management come to pharmacy as well. Pt said she was fine to go, but I told her to have a seat in the waiting area for at least 20 minutes so we can keep an eye on her. Told her in the future to indicated that she has fainted from a vaccine.
