NO ADVERSE REACTION. (ADVERSE REACTION DATE IS A REQUIRED FIELD ON PAGE ONE) NO ADVERSE REACTION. Incorrect vaccination given. Patient received adult Hep A vaccination instead of 0.5 ml child dose. patient reported concerns or side effects. Patient had two doses previously but first dose noted invalid in the system as it was given by old PCP office a few days prior to 1st birthday but old PCP advised it was valid and after second dose patient was complete with that series per father. Our office gave not only a extra hep A because we did not diligently review the WIR and simply reviewed health maintenance in our EHR but ALSO associate grabbed and administered the adult dose Harvix 1.0ml. Patient pulled away during administration, Associate swapped needle for new needle and gave remainder of vaccine. Initially associate was concerned as patient only received an estimated half of the dose due to waste during difficult administration and when applying the new needle but upon review and documentation of the administration discovered it was the wrong formulation for age and that there was two administrations that a provider had found valid in the past. We discussed with patient parent and no adverse reactions or symptoms were present. We monitored child for 15 minutes and again asked their status 72 hours later.
