03/13/26 CST: 09:20AM Patient stated she was scared and nervous for vaccines. Education provided over how vaccines would be administered, symptoms she may experience following the vaccines, and signs/symptoms to report to healthcare professional. 09:22AM Patient was administered Menveo in right arm. Patient tolerated well. 09:23AM Patient stated she was nervous the next shot would hurt. 09:24AM Patient was administered Tdap in left arm. Following administration, patient became pale. When asked if she was feeling okay, she said she felt dizzy and vision was dark. Patient was fanned. Given a sip of soda. 09:26AM Patient stated she was nausea, had 2 emesis events. Stated she felt a lot better just feeling a little shaky. 09:27AM Guardian was contacted. Stated she would try to send grandmother to pick up. 09:30AM Stated she felt better, still just a little shaky and dizzy. Wet cloth applied to skin. Color slowly returning. 09:35AM Patient started drinking soda and eating a bag of chips. 09:50AM Patient stated she was feeling better and ready to return to class. Walked back to class. Stated she still felt a little weak. Instructed to return if symptoms continued or worsened.
