Error : Medical Asst. added diluent/sterile water to the Meningococcal Conjugate Quadrivalent (MenQuadfi) vaccine. Per CDC guidelines vaccine invalid due to diluent added and is not required . CDC Guidelines recommend to re- vaccinate Meningococcal Conjugate Quadrivalent (MenQuadfi) vaccine. Pt. was re- vaccinate with no adverse reaction at the time of vaccination error.
