The patient's medical history, concurrent condition, patient's past medications , concomitant medications were not reported. On 04-Mar-2026, the patient was vaccinated with first dose of human papillomavirus nonavalent (types 6, 11, 16, 18, 31, 33, 45, 52, 58) (GARDASIL 9) injection, 0.50 mL (Lot# y019751 has been verified to be a valid lot# Y019751 expiration date: 16-Feb-2027, dose text: product quantity: 1; strength and route was not reported for prophylaxis (product administered to patient of inappropriate age). It was verified that no side effects or symptoms had been reported and stated that the patient may not receive another dose of human papillomavirus nonavalent (types 6, 11, 16, 18, 31, 33, 45, 52, 58) (GARDASIL 9) after his initial vaccination but could not confirm at that time.
