Symptom narrative

The site received a vaccine shipment two days after the stated ?must be received by? date. Upon arrival, the vaccines were cold to the touch and the ice packs were still frozen. As a precaution, the vaccines were placed in the freezer, labeled ?Do Not Use,? and we initiated the process to replace them. Unfortunately, before the replacement process was completed, a nurse inadvertently administered these vaccines to this patient. Manufacturer was notified as well as DPH. We consulted with DPH Vaccine Coordinator, and he said that since Merck stated the vaccine was not viable, then the patient would need to be revaccinated. Patient will need to wait 28 days from when they were vaccinated with the non-viable dose to re-vaccinate. Patient's parents were contacted to report error and need to revaccinate.
