Symptom narrative

Patient signed consent to receive Gardasil Vaccine and received it on her Left deltoid. Patient went to restroom and provided urine specimen and returned back into the room and sat in the chair asked if she could sit for few minutes as she was feeling dizzy. Asked patient if she wanted to lay down and she said yes as she was "going to faint" patient laid down and then fainted on the exam table. I called for a RN and RN came into the room. RN found patient with eye twitching, witnessed arms flailing and was unresponsive for a few minutes. Patient awoke and said she was nauseous but did not vomit. Provider was called, vital signs were taken, patient was positioned on Left side awaiting EMT arrival fingerstick BS was 84. Patient reported she was fasting for blood work prior to appt. O2 was 98 HR was 64 Patient was noted to have episodes of eye twitching 2-3x while waiting for paramedics.

Current illness

chronica arthralgia, migraines, generalized anxiety disorder and PTSD, autonomic dysfunction syndrome due to EDS, recurrent bronchitis and pneumonia

Medical history

PTSD generalized anxiety disorder migraine, chronic arthralgia, EDS back pain autonomic dys

Tylenol 500 mg, epi pen 0.3 mg/3 ml, nabumatone 500 mg, omeprazole 40 mg, ondansetron 4mg, pyridostigmine 60 mg, venlafaxine ER 150 mg , zolmitriptan 5 mg,

Allergies

peaches, prunus percica, amoxicillin, fish, mushrooms and mushroom extract, penicillin, pineapple and Topamax
