Symptom narrative

The patient was brought in by mom after noticing a rash on the right thigh yesterday morning next to the vaccine site. The patient was examined today and has no symptoms, no fever, and is eating well. The mom was advised to continue to observe rash and to call back if the patient experiences fever or notices the rash continues to spread.
