Symptom narrative

Client became pale and diaphoretic approximately 15 minutes after receiving HPV vaccine. At 1500, vital signs were BP 87/65 and pulse 80. Client was monitored and remained alert and oriented. At 1530, client was discharged in Mom's care in stable condition, alert and oriented, with vital signs BP 87/62and pulse 81.
