Symptom narrative

Mom was here with child for shots, older child (brother) interpreted [withheld] at 10:00 am on 02/25/2026. NKA. Pt stated he had not eaten anything that day, gave sprite and pack of peanut crackers. Pt did not drink the soda but ate the peanut crackers. At 10:15 MenACWY-TT was given in right deltoid. Was pt 2nd does. Pt stated he didn't smoke but some friends did. Pt appeared to have tolerated will but within 5 minutes pt complained of left side of face being numb, then stated the vision in left eye becoming blurred and lightheaded. Called RN, DON, she came and helped move pt to bed. Called 2nd nurse, RN. Ammonia used. 1st BP: 96/60, HR: 58, Sat: 97%. 2nd BP: 102/62. Pt stated he was feeling better. Pt set up on bed and stated he was feeling good. Attempted to walk pt out to lobby at 10520 and didn't get out of room to hall and he stated he was feeling light headed again. Set pt in chair. BP: 90/60, HR: 54, Sat: 98%. Pt became non-responsive, Ammonia, and sternal rubs used. Ambulance called, pt was taken to ambulance per wheelchair and taken to Medical Center with mother at side.
