Symptom narrative

Pt stated yesterday 3/12/26 area was sore to the touch and she had warmth at site and ow grade temp, she took otc dayquil , no tylenol. She reports that today 3/13/26 it has become itchy, the red circle on left deltoid measures 20mmx 30mm and has hard center in middle. Pt denies any shortness of breath or difficulty breathing. she denies any dysphagia. she stated in the past she has always had pfizer covid shots and never had any problem . this was her first time receiving moderna.
