Symptom narrative

The patient returned on 03/13/2026 at 11:30 AM for TB skin test reading. During the visit, the patient?s mother reported that the patient continued to experience pain in the right arm following the immunizations. The LVN assessed the injection sites and observed redness and a wheal on the upper right arm around the deltoid area. The patient reported itchiness at the site and stated that he had been scratching his arm. Upon further assessment, the patient reported that the itchiness began the day after receiving the vaccinations. Due to the reported symptoms and observed localized reaction, the LVN advised the patient?s mother to seek medical evaluation at an urgent care center or emergency department. A referral list of healthcare providers who accept patients without insurance was provided. The family was instructed to seek immediate medical attention if symptoms persist or worsen. An immunization record was provided to the family with the vaccine sites, vaccine names, and lot numbers highlighted for reference by the evaluating healthcare provider. The family has limited language proficiency; therefore, the LVN requested assistance from medical facility from the TB clinic to provide translation during the encounter. Supervisor notified.

No medical tests were done, medical facility is only a vaccination clinic. LVN advise parent to seek medical attention for further evaluation from Emergency department or Urgent Cart. Referral list to other medical provider was also provided to parent.
