Administered on Right Deltiod. I went to prep immunizations and grabbed the Dtap then realizing we had Kinrix in stock i did not place the Dtap back in the fridge and proceeded to get the rest of the medications and proceed with preparations. Then after i administered Immunizations, did not get the chance to document until after done with patients around 12:10 then i began to document and realized that the Dtap i had gotten first was in my tray as an extra dose since that was the last one i had to document i then had to clock out for lunch. As soon as i clocked out i went to my supervisor and notified her since she was on lunch also. I needed to be advised. Provider had previously ordered all individuals but considering patient i asked mother if it was okay for combos for less sticks for patient since she already had previous dosages, and no adverse reactions. Then after i asked provider if per mom we can reorder them in combos. In between the change of the individuals and combos i made the mistake and gave the extra dose of Dtap how i mentioned i had gotten the individual first did not put it back in the fridge after i grabbed the Kinrix.
