Shortly after vaccination, patient reported dizziness, shivering, and brief fainting episode lasting a few seconds. Patient also complained of headache, but stated later that the headache was subsiding. Patient reported not having eaten yet today prior to vaccination. 4:17 PM Initial vital signs: SpO2: 97% Pulse: 64 bpm Temperature: 97.1 °F Patient observed to have brief syncopal episode with shivering and dizziness. Patient given water and crackers. 4:27 PM Reassessment: SpO2: 99% Pulse: 94 bpm Temperature: 97.04 °F BP: 108/84 mmHg 4:50 PM BP reassessed: BP: 104/64 mmHg Patient alert and responsive after episode. Symptoms improving. Patient condition improved after rest and oral intake. Vital signs stabilized. Patient monitored until vital signs stabilized. Provided oral fluids and crackers due to reported fasting. Advised patient's mother to seek ER care if headache persists or symptoms worsen. Plan to conduct follow-up call with patient tomorrow to reassess condition. Patient left the clinic in stable condition.
