only given the adjuvant component of Shingrix instead of the reconstituted dose; only given the adjuvant component of Shingrix instead of the reconstituted dose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate preparation of medication in a 68-year-old male patient who received Herpes zoster (Shingrix) for prophylaxis. On 02-MAR-2026, the patient received Shingrix. On 02-MAR-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: only given the adjuvant component of Shingrix instead of the reconstituted dose) and inappropriate dose of vaccine administered (Verbatim: only given the adjuvant component of Shingrix instead of the reconstituted dose).
